LWC is looking for an experienced Home Care Aides Certified Nursing Assistants for our Residential Treatment and Assisted Living facilities. These positions are full-time. Position will be responsible for assisting our residents with daily activities including transferring and toileting. Position may require working in different facilities as needed on a floating basis.

Minimum Qualifications:

Home Care Aide/Certified Nursing Assistant credentials from WA state Department of Health in good standing.

A High School Diploma or equivalent, plus experience in work with special needs populations, AA or BA in Human Services related disciplines preferred.

Good organizational skills, ability to work independently and manage multiple tasks.

Ability to work cooperatively in a team approach;

Ability to apply a Recovery based approach and to apply clinical knowledge;

Good verbal and written communication skills, basic computer skills.

Ability to exercise independent judgment and discretion;

Visual and auditory skills required to communicate, to ensure and deliver client care, and to provide documentation of care;

Requires frequent standing, bending at the knees and waist, kneeling and reaching; Requires frequent walking to accomplish tasks; Requires ability to lift 50 pounds.

Ability to obtain First Aid/CPR, and successful completion of Boarding Home training requirements (WAC 388-112).

Employment is contingent on successful fingerprint background check.

The lake Whatcom Center is seeking a dedicated and compassionate Case Manager, fluent in English and Spanish, to join our team.

As an Intensive Outpatient Case Manager, you will play a vital role in providing support and guidance to clients in need. If you have experience in social work, behavioral health, and case management, we encourage you to apply. Responsibilities:

- Must be 21 years of age or older.
- Conduct needs analysis with clients and develop individualized care plans
- Collaborate with healthcare professionals to coordinate and facilitate client care
- Provide education and support to clients
- Monitor client progress and adjust care plans as needed
- Assist with care coordination and ensure appropriate follow-up care
- Advocate for client's rights and ensure they receive necessary resources and services
- Maintain accurate and confidential client records in compliance with HIPAA regulations
- Utilize case management software to document client interactions and track progress

- Maintain a minimum productivity ratio of 40%

Experience:
- Associate's, Bachelor's, or Master's degree in social work or related field
- Previous experience in case management, preferably in a healthcare setting
- Knowledge of pediatrics, behavioral health, and addiction counseling
- Familiarity with utilization management principles
- Strong communication skills, both written and verbal
- Ability to work effectively as part of a multidisciplinary team
- Excellent organizational skills and attention to detail
